CHLORAL HYDRATE / HYDROXYZINE의 경구투여와 MIDAZOLAM의 경구 및 비강내 투여의 의식진정효과에 관한 비교연구

Abstract
결론
저자들은 전남대학교 병원 소아치과에 내원한 환아중 첫 내원시 간단한 구강검사에도 행
동조절이 어려운 광범위한 수복치료가 요구되는 Frankl score 1과 2에 해당되는 비협조적인
아동에 약물투여를 시행하였다. 대상아동들은 전신적으로 건강이 양호하고 나미 16-87개월,
제충 11-35kg의 60명의 아동이었다. 자정부터는 금식하도록 지시하고 무작위로 선별하여 진
정제 약종류에 따라 3군으로 나누어 시행하였는데 제 1군은 Chloral hydrate 75mg/kg 및
Hydroxyzine 25mg을 경구투여후 45분, 제 2군은 Midazclam 0.5mg/kg을 10ml의 과일쥬스
에 섞어 경구투여후 30분, 제 3군은 Midazolam 0.3mg/kg을 비강내 투여후 10분간 조용한
곳에서 대기하도록 한 후 drowsy상태가 되면 진료실로 이송하여 Pedi wrap으로 신체를 속
박한 후 Pulse oximeter sensor를 부착하여 SaO2와 맥박수를 측정하였고,
dental chair와 pediwrap에 위치시키는 순간, 마취시기, 러버댐장착시기, 와동형성 시기, 충
전시기로 나누어 수면, 울음, 움직임에 대해 평가하고 치료완료후에는 전반적 행동을 평가하
여 의식진정효과에 대해 다음과 같은 결론을 얻었다.
1. chloral hydrate/hydroxyzine 경구투여군의 sleep score가 midazolam 경구투여군과
midazolam 비강내투여군에 비해 유의하게 높았다(p<0.05).
2. 처음 치료시작시에만 chloral hydrate/hydroxyzine 경구투여군이 crying과 movement에
서 midasolam 비강내투여나 경구투여군보다 유의하게 높은 score를 받았으나 다른 술식동
안에는 유의한 차이가 인정되지 않았다(p>0.05).
3. Overall behavior는 chloral hydrate/hydroxygine 경구투여군에서의 mean score가 3.25
±0.79, midazolam 비강내 투여군에서 2.8±1.1, midazolam 경구투여군에서 2.6±0.82로
chloral hydrate/hydroxyzine 경구투여군에서 가장 높았으며 성공율도 각각 80%, 60%, 60%
로 chloral hydrate/hydroxyzine 경구투여군에서 가장 높았으나 유의한 차이는 인정되지 않
았다(p>0.05).
4. 청색증이나 호흡억제 등과 같은 부작용은 관찰되지 않았으며 대체로 안정된 맥박수와
동맥혈 산소포화도가 관찰되어 임상적으로 안정된 생징후를 보였다.
본 임상연구의 결과로 미루어보아 chloral hydrate/hydroxyzine경구투여군이 midazolam군
에 비해 소아치과환자의 행동조절을 위한 더 바람직한 overall behavior를 보이는 진정요법
이며 midazolam경구 및 비강내투여군도 소아치과 환자에게 비교적 안전하고 효율적인 진정
요법으로 여겨진다.

The purpose of this study was to assess the sedative effect of three kinds of
medication for management of the uncooperative 60 children aged from 16 months to 87
months required extensive treatment. The patients were given randomly a dose of
75mg/kg of chloral hydrate 3nd hydroxyzine 25mg orally or 0.5mg/kg of midazolam
orally or 0.3mg/kg of midazolam intranasally. All the children were rentrained in a
Pediwrap and were monitored with pulse oximeter for assessing the safety of patients.
According to rating scale, sleep, crying, movement, and overall behavior were checked
for evaluation of the clinical sedative effect.
The results were as follows :
1. In the evaluation of sleep, rating scale of chloral hydrate/hydroxyzine was superior
to the other group(p<0.05).
2. In the evaluation of crying and movement at beginning of treatment, rating scale of
chloral hadrate/hydroxyzine was superior to the other group(p<0.05), but during the
treatment, rating scale of each group was not significantly different (p>0.05).
3. In the evaluation of overall behavior, 80% children of chloral hydrate/hydroxyzine
were rated good or very good. With the oral and intranasal midazolam, 60% children
were rated good or very good respectively.
4. There were no clinical signs of significant cyanosis and respiratory depression.
Clinically, chloral hydrate/hydroxyzine was proved to be more effective sedation than
any other sedative method. Oral and intranasal midazolam were also safe and effective
sedation in young children undergoing pediataric dental procedures.
